The Denali NAND controller IP needs three clocks:

 - clk: controller core clock

 - clk_x: bus interface clock

 - ecc_clk: clock at which ECC circuitry is run

Currently, only the first one (50MHz) is provided.  The rest of the
two clock ports must be connected to the 200MHz clock line.  Add this.

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from.  So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry.  Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.

In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.

This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents.  There's also a few places where I found we did not have a tag
and have introduced one.

The last clock consumer in SPL, SD/eMMC driver, gave up using the
clock driver.  The clock driver is only used in U-Boot proper.

U-Boot does not support fancy clock tree structures like the Linux
common clock framework.  Implement a simple clock tree model at the
driver level.  With this, the clock data will be simplified.

Since commit f0776a551764 ("spl: dm: Kconfig: SPL_CLK depends on
SPL_DM"), the following warning is displayed:

  $ make uniphier_v8_defconfig
  warning: (ARCH_ZYNQ &amp;&amp; ARCH_ZYNQMP &amp;&amp; STM32F7 &amp;&amp; CLK_UNIPHIER) selects
  SPL_CLK which has unmet direct dependencies (CLK &amp;&amp; SPL_DM)

While I am here, I am removing the prompt to make it user-unconfigurable
option so that "select CLK_UNIPHIER" can be omitted.
